1: Domestic - Family Violence Screening: Use this form if the courts/probation have said you need to attend a Family Violence Intervention Program. Victim is usually a spouse, girlfriend, boyfriend, child or parent.

2. Anger Management Relationship Screening: Use this form if the courts/probation marked "anger management" and the victim is a spouse, girlfriend, boyfriend, child or parent.

3. Anger Management Evaluation: Use this form if the victim is a stranger, neighbor, police officer, co-worker.

4. Alcohol and Drug Evaluation: Use this form if the courts, probation, or attorney said you need either a alcohol or drug evaluation.

Domestic Violence, Anger Management Counseling:

Certified Treatment Provider for Domestic Violence Intervention and Anger Management Classes. Men’s and Women’s classes available. State certified by the Georgia Commission on Family Violence and supervised by the Department of Corrections. Evening and weekend classes available.

Alcohol and Drug Evaluation:

Clinical Evaluator on the State of Georgia’s DHR Registry for individuals seeking an evaluation for driver’s license reinstatement as well as for Probation.

Alcohol and Drug Treatment/Education:

Treatment Provider for the State of Georgia’s DHR Registry for ASAM Level I. Both individual and group counseling available. Alcohol and drug education classes and ASAM Level I group treatment.

Substance Abuse Professional:

Certified through NAADAC (#10282) and DOT to perform clinical evaluations.

Addiction Counselor Supervision:

Supervision available for Certified Addiction Counselors seeking recertification as well as those seeking initial certification. Georgia Addiction Counselors Association Certified Clinical Supervisor (CCS) # 0199.

Family Violence/Anger Management:

Provider for Family Violence Intervention Programs and Anger Management. Classes are provided for those who have been convicted of family violence and are court-mandated as well as those who want to enter voluntarily.

Pastoral Counseling:

Death, Grief and Loss; Faith and Conflict; Spirituality; Relationship Issues; Emotional, Physical, and Sexual Abuse; Adolescents and Children; Intimacy Issues; Divorce Recovery; Caretaker Issues.
